Pre-test Activity 2 (Case: Steph and Dylan) - Discharge Summary documentation

In this activity, you will need to:

1.      Open and review the documents related to Dylan’s admission (including your ward round progress note document from Pre-Test Activity 1) - this is your patient’s (Dylan’s) medical record.

2.      Document a discharge summary (clinical handover) to Dylan’s general practitioner (Primary Care team) into the space below.

YOU WILL BE GIVEN ACCESS TO THE MODULES AFTER COMPLETION OF THE PRE-TEST
